CTI Toolbar Does Not Appear - With Message "User Has Maximum Communication Toolbar Sessions" (Doc ID 489496.1)

When a user logs into Siebel with CTI enabled and then closes the browser before logging out, the next time the user logs in, they get the following error:

"Communication: User has maximum Communication toolbar sessions."

It seems as though the Toolbar session is not releasing. How do we resolve this issue?
