Why Workflow policies not getting triggered? (Doc ID 503856.1)

Symptoms

When Workflow Policies are no longer being triggered, a high number of records are seen in QUEUED status in the S_ESCL_REQ table.

These "queued" requests in S_ESCL_REQ table do have a valid RULE_ID and GROUP_ID, however the Workflow Monitor Agent stops picking up the requests for processing.
