Why Workflow policies not getting triggered?

Workflow Policies, when implemented, are critical to a business as these policies call Workflow Processes to run critical processes.
When Workflow Policies are no longer being triggered, you may see a lot of records in QUEUED status in the S_ESCL_REQ table.
These "queued" requests in S_ESCL_REQ table have a valid RULE_ID and GROUP_ID, however the Workflow Monitor Agent just stopped picking up the requests for processing.
