ddlsync trying to add existing column (Doc ID 509389.1)

Symptoms

I launch the synchrinize physical schema from the configure db server wizard and it fails when trying to add columns (some are exension columns some are vanilla comumns) to a table when they already exist. I have attached the log file. Please note that I have verified the succesfull completion of
- grant user, (the SIEBEL user is the dbo in siebeldb)
- install Siebel Database
- imprep of vanilla repository
- rename of Siebel repository
- imprep of custom repository (validated the exprep that produced the dat file)

The same .dat file has been imprep in an other environement and the ddlsynch ran witout any problems. Please note that we are using patch 23 on all of our environements. I have dropped every table and started over with the same results. Error in the ddlsynch2.log file:

S0021: [Microsoft][ODBC SQL Server Driver][SQL Server]Column names in each table must be unique. Column name 'EXT_ATTRIB_07' in table 'EIM_ACCNT_DTL' is specified more than once.
