SAM-QFS: Drive Initialization Fails With "Unable to determine media type" Error (Doc ID 1008495.1)

Symptoms

When attempting to initialize the library and robot, the drives immediately enter the "down" state and the error "Unable to determine media type" is displayed in the sam-log or console.

The /var/opt/SUNWsamfs/devlog/<eq id> device log shows "Error: Unknown device":

Symptoms:

Drive is downed
Tapes will not mount
Files are not archiving
Files are not staging
File system is 100% full
