Ultra Enterprise[TM] 3500: Resolving intermittent internal boot disk failures (Doc ID 1008780.1)

Symptoms

 Booting from an Ultra Enterprise 3500 internal drive fails with "invalid wwn" and "can't open boot device". The boot drive, as well as all drives on the boot drive's fibre loop cannot be seen with probe-fcal-all at this point. A subsequent power cycle typically allows the system to boot.

Note: This boot failure may occur intermittently. (i.e.; it may take several successful reboots before a failure reoccurs).
