Emulex driver 'emlxs' reports "Packet timeout" and may cause PCI panic (Doc ID 1011035.1)

Symptoms

Symptoms

A host configured with an  Emulex HBA can generate packet timeout messages. Following is an example of the message.

This will cause IOs to get retried, which may impact the performance of the system. This might also cause occasional IO failure when the system is under heavy load.

Also, in some cases it is found that system may panic. Following is an example
of the panic.
