Solaris Cluster 3.x samfs-qfs File System Fails to Mount with Error "mount_samfs: mount() error: Device busy" (Doc ID 1012872.1)

Symptoms

When booting a cluster node into a Solaris Cluster 3.1/3.2/3.3 configured with SAMFS-QFS, or when trying to start the QFS resources on any of the cluster nodes, the shared qfs file systems fails to mount.
This can happen on all cluster nodes running SAMFS-QFS, or can happen on any one node but mounts fine on the metadata server.
