ACSLS - Database Failure - ACSLS Will Not Start Up

Symptoms
On an ACSLS 7.x system, the following symptoms may exist:
- "EXEC SQL open lsm_cursor" error in the acsss_event.log
- various STATUS_DATABASE_ERROR and/or STATUS_PROCESS_FAILURE messages in the acsss_event.log
- There is no sm_versions file in $INFORMIXDIR/etc directory as indicated in the lib6_bar_act.log
- Backups have been failing since after the upgrade.
