Symptoms
Solaris command "newfs" encounter the following I/O error resulting failure of execution.
# /usr/sbin/newfs <raw-device>
newfs: construct a new file system <raw-device>: (y/n)? y
read error on sector XXXXXXXX: I/O error
where <raw-device> could be any raw devices which include the followings :
(a) Solaris[TM] raw device /dev/rdsk/c#t#d#s#
(b) Solstice DiskSuite[TM] (SDS) OR Solaris[TM] Volume Manager (SVM) raw device /dev/md/rdsk/d#
(c) Veritas Volume Manager (VxVM) raw volume /dev/vx/<DG-name>/<volume-name>
EXAMPLES :
# /usr/sbin/newfs /dev/rdsk/c0t0d0s5
newfs: construct a new file system /dev/rdsk/c0t0d0s5: (y/n)? y
read error on sector 25467843: I/O error
# /usr/sbin/newfs /dev/md/rdsk/d1
newfs: construct a new file system /dev/md/rdsk/d1: (y/n)? y
read error on sector 42598304: I/O error
# /usr/sbin/newfs /dev/vx/rdsk/drk1tr7a/v01
newfs: construct a new file system /dev/vx/rdsk/drk1tr7a/v01: (y/n)? y
read error on sector 30924799: I/O error
NOTE : It is important to note that the root cause of the problem is NOT due to
storage software SDS/SVM OR VxVM .
Changes
Possibly either
A Storage hardware failure has occurred
or
An Availability suite device is in the offlined state
