Solaris Cluster SMF service ql_upgrade does not start at boot (Doc ID 1312274.1)

Symptoms

Solaris Cluster node joins cluster however some Solaris Cluster SMF services are not starting:

offline 11:33:42 svc:/system/cluster/mountgfs:default
offline 11:33:45 svc:/system/cluster/clusterdata:default
offline 11:33:45 svc:/system/cluster/ql_rgm:default
offline 11:33:45 svc:/system/cluster/scsymon-srv:default
maintenance 12:02:08 svc:/system/cluster/ql_upgrade:default


Cluster resources are also blocked from starting:

/var/adm/messages

Apr 8 09:31:44 node1 Cluster.RGM.global.rgmd: [ID 670814 daemon.notice] Blocking in RGM

errors in SMF log for ql_upgrade
