Symptoms
MVC is stuck as being offsite in the Vault in the CDS VAULT record.
The MVC cartridge is destroyed and cannot be entered into the ACS to clear the vault status.
Vault volume has a vault return date so LCM reports it needs to be returned from the vault, however the tape is either lost or damaged and cannot be entered into the ACS.
