Qlogic QLA2340 card goes offline in an X4200, X4100 server (Doc ID 1332020.1)

Symptoms

Qlogic cards goes offline and does not return until after a reboot.

The log file /var/adm/messages shows errors similar to the following:

Mar 17 14:43:19 fctl: [ID 517869 kern.warning] WARNING: fp(1)::N_x Port with D_ID=610000, PWWN=200500a0b818747e disappeared from fabric
Mar 17 14:50:57 fctl: [ID 517869 kern.warning] WARNING: fp(1)::N_x Port with D_ID=610000, PWWN=200500a0b818747e reappeared in fabric

Changes

This particular problem involves Qlogic QLA2340 card, installed in an X4200 and possibly an X4100 server
This issue is not present in the M2 variant of the platform.
