SAM-QFS: samfsck Fails With "no quota record for index" (Doc ID 1339223.1)

Symptoms

In some occasions, a samfsck(1M) can fail with the warning "no quota record for index"
Ex :

# samfsck -V large
[..]
  samfsck: group: no quota record for index 38050
  samfsck: group: no quota record for index 36060
  samfsck: group: no quota record for index 34330
  samfsck: user: no quota record for index 55017

[..]

Changes

This situation can  occur when the quota records (.quota_a,  .quota_g or .quota_u) were created for an existing sam-qfs file systems (as decribed in  How to Configure an Existing File System to Use Quotas) and files were modified or added with an higher adminID, groupID or userID then the highest possible record defined during the creation of the quota file(s); these file modifications took place while the file system was mounted with quotas disabled.
