Symptoms

Two node cluster where both nodes panic at the same time with

    panic string:   CMM: Cluster lost operational quorum; aborting.

And messages on both machines report

Oct  3 21:56:04 Node04 cl_runtime: [ID 841616 kern.warning] WARNING: CMM: This node has been preempted from quorum device /dev/did/rdsk/d8s2.
Oct  3 21:56:05 Node04 cl_runtime: [ID 266834 kern.warning] WARNING: CMM: Our partition has been preempted.

Oct  3 21:56:04 Node03 cl_runtime: [ID 841616 kern.warning] WARNING: CMM: This node has been preempted from quorum device /dev/did/rdsk/d8s2.
Oct  3 21:56:05 Node03 cl_runtime: [ID 266834 kern.warning] WARNING: CMM: Our partition has been preempted.
