After patching, existing iscsi target is no longer recognized and instead a new iscsi target is enumerated (Doc ID 1388884.1)

Symptoms

The host sees the target iscsi device as say, c2t3d0

c2t3d0 <NETAPP-LUN-0.2 cyl 6142 alt 2 hd 16 sec 640>
        /iscs/disk@0000iqn.1992-08.com.netapp%3Aemomni0048c02%3AecsmcFFFF,0

The iscsi information from the target shows:
Target: iqn.1992-08.com.netapp:emomni0048c02:ecsmc
      Alias: -
      TPGT: 2027
      ISID: 4000002a0000
      Connections: 1
      LUN: 0
           Vendor:  NETAPP
           Product: LUN
           OS Device Name: /dev/rdsk/c2t3d0s2

After patching the system, it now sees the same target as c2t4d0

Nothing has changed on the target side.
