SL8500/SL3000 - Drive Load Failed Due to Media Error - On Failed Load

Symptoms
Tapes are getting mounted but load operation completes with error.
Tapes can be dismounted with the force option in ACSLS or HSC.
During a load operation, the library reports error 6415 - "The load failed and the cartridge is still present"
