SL8500/SL3000 - Reported 3337/3357 Error Code Over Threshold Due to Tape Leader Problems (Doc ID 1474748.1)

ASR Problem description shows:
     The Oracle Proactive Support Center has reported that serial number 516000200100 has exceeded the daily error threshold for event 3337/3357 on the following volsers.
     Exceeding this threshold may indicate a failing cartridge or drive in the SL8500/Sl300 library. Please follow the RDC process to diagnose whether this is a drive failure or a
     suspected cartridge and dispatch to the field if necessary.

A drive reports "load failed" errors followed by "rewindUnload Successful with mediaError" warnings.
For example:

      2012-07-06T13:53:30.227 error 3955 "Error from device Code: 605 - Drive media error"...<resultCode>6415</resultCode><resultText><!|CDATA|drive 1,2,2,1,4: load FAILED with mediaError: CartLabel

      2012-07-06T13:53:30.251 error 3337 "Drive load failed due to media error - on failed load", volumeLabel=B11193T1 hliLsm=1

      2012-07-06T13:53:30.449 warn 3992 "Warning from device", Data=<response sequence="5768152" final="true"><command>rewindUnload</command><result identifier="1"><resultStatus>
           <resultSeverity>warn</resultSeverity><resultCode>6434</resultCode><resultText><!|CDATA|drive 1,2,2,1,4: rewindUnload Successful with mediaError: Cartridge Label: B11193T1,  FSC: 5135||></resultText>


At times, these warning messages may also be reported:

      Warning from device, drive 1,1,2,1,1: load Successful with mediaError: , fsc: FSC: 5A51 (sn = 576004001247)
      Warning from device, drive 1,1,2,1,1: rewindUnload Successful with mediaError: CartLabel , FSC: 5A51
