SAM-QFS: Clients Unable To Read A File They Just Wrote, Getting I/O errors (Doc ID 1500099.1)

Symptoms

Example of configuration:

There are 2 nodes configured in Solaris Cluster and working as Oracle RAC cluster nodes.
The filesystem is QFS shared filesystem. Apart from the Database filesystems, there are 2 filesystem /qfs1 and /qfs2 are configured to share it with both the cluster nodes and 2 application nodes outside of cluster using CTOC (Clients Outside The Cluster) option.
 
cluster nodes name:
node1
node2

filesystem qfs1 and qfs2 are also mounted on client hostname client1

mcf file on both node1 and node2
