Symptoms

 You are trying to log into the GUI and after a number of unsuccessful attempts, the GUI login screen is showing:

For the Axiom software version 4.x browser-based GUI -- Administrator account is disabled.

For the Axiom software version 5.x java-based GUI -- a window pops up with text "Authentication failed. Please contact your Administrator to verify account credentials."
