Symptoms
fsck might fail for UFS filesystems on EFI labelled devices under certain conditions:
- filesystem has been created initially on physical slice (partition, e.g. /dev/rdsk/c0t0d0s0)
- metadevice (SVM) was created (metainit) *after* the mkfs/newfs on physical slice
