We have identified a bug in Ops Center 12.1.4 that can cause problems communicating with assets. A symptom of this problem is a warning level incident with initial description "Management access point failed authentication during login to asset. Access point it blacklisted from future connections to prevent security lockout." Later the description is prepended with "Increased severity as incident repeated 50 times".
For example if you add an asset to Ops Center - first ILOM, then installed Ops Center Agent to OS. Into logs information you will see that Ops Center is still trying to connect to this asset via SSH even if SSH is not configured in Ops Center.
Also unconfigure agent from server doesn't solve the problem. OC is still trying to connect to the server via SSH protocol.
