Symptoms

The /var/adm/messages file will have the following events logged:

Sep 19 04:09:46 hostname fp: [ID 517869 kern.info] NOTICE: fp(3): PLOGI to 48004f failed state=Timeout, reason=Sequence Timeout
Sep 19 04:09:46 hostname fctl: [ID 517869 kern.warning] WARNING: fp(3)::PLOGI to 48004f failed. state=c reason=2.
Sep 19 04:09:57 hostname fp: [ID 517869 kern.info] NOTICE: fp(5): PLOGI to 48004f failed state=Timeout, reason=Sequence Timeout
Sep 19 04:09:57 hostname fctl: [ID 517869 kern.warning] WARNING: fp(5)::PLOGI to 48004f failed. state=c reason=2.


Using the above example, we observe the HBA is receiving the  RSCN (Registered State Change Notification) from the Fabric Controller on the SAN switch for port 48004f. The fp driver, upon receiving such RSCN, will issue the PLOGI (Port Login) to such port, which is returning a failure/timeout condition. Further investigation into the output of "luxadm -e dump_map" shows the following:
