Symptoms

The following errors are seen when attempting to boot from the internal disk to Solaris:

 

{0} ok boot disk0
Boot device: /pci@400/pci@2/pci@0/pci@e/scsi@0/disk@p0  File and args:
LSI MPT chip is reporting FAULT state with code 8006
ERROR: boot-read fail


Can't open boot device

 

 Running probe-scsi-all from the ok prompt shows the following message:

{0} ok probe-scsi-all

/pci@400/pci@2/pci@0/pci@e/scsi@0
LSI MPT chip is reporting FAULT state with code 8006
Can't open adapter

 These fault messages have been seen on SPARC T3-1, SPARC T3-2, SPARC T4-1, SPARC T4-2, Netra T4-1 and Netra T4-2
