Symptoms

The T6300 panics & then cannot reboot:

The / file system (/dev/rdsk/c1t0d0s0) is being checked.

Repeating drive offline messages were shown on target 2 and server didn't boot further

       drive offline

 

The OBP also cannot access the disks:
