Server Rebooting After A Disk Failure In Raid 10 Array, panic in cpqary3 driver (Doc ID 1641506.1)

Symptoms

We have had an issue with an HP DL380 server, the server has 4 x 146GB disks in a raid 10 for OS and 4 x 600GB disks in Raid 10 for data.
We had a disk fail in the data raid set, this caused the server to go into an endless reboot until the disk was replaced.

Panic stack indicates a problem in cpqary3_NOE_handler() function.
