Symptoms
Storage team reconfigured the Fabric zoning but the issue persisted (cfgadm -al still did not show the controller numbers for the NPIV ports).
In order to workaround the problem we tried this:
Deleted all the NPIV ports from control domain Solaris 11.1 (SRU 0.5.11-0.175.1.14.0.5.0) server1 (fcadm delete-npiv-port)
Then rebuilt all the devices and path_to_inst (eg. removed and boot reconfigure so that everything is clean).
Next, created a new NPIV port with a different WWN in which the Fabric did not have zoning for it.
The objective of the test was to isolate the dependency on Fabric config.
Then, performed the same steps on T4-2 Solaris 11.1 (SRU 0.5.11-0.175.1.0.0.24.2) server2.
The result showed that in server2, after creation of the NPIV port, the controller number appeared in cfgadm -al command.
But in server1, after creation the NPIV port, the controller number did NOT appear in cfgadm -al command.
It seems that the issue is not related to SAN side (eg. zoning or array config).
Note that server1 has an I/O domain and the PCIe buses were splitted for the I/O domain.
