Symptoms
This example is from a M5000 SPARC Solaris server but will apply to all servers and Solaris x86 as well.
Issue
- Second port of fc hba card in slot location /devices/pci@2,600000:iou#0-pci#3 is not seen by Solaris and appears to be defective.
First port of the card appears normal.
Seeing Fibre Channel (FC) HBA card messages "Adapter reset failed." "Adapter initialization failed."
