Fibre Channel (FC) SAN-Connected Storage Devices Disappear After a Non-Related Server Part is Replaced

Symptoms
A system board, a backplane, or some other internal component of a server is replaced, necessitating removal of all cables connected to the server. After the part has been replaced, and cables have been reconnected, the FC/SAN devices have all disappeared:
luxadm -e port
/devices/pci@8,600000/SUNW,emlxs@1/fp@0,0:devctl NOT CONNECTED
/devices/pci@8,600000/SUNW,emlxs@2/fp@0,0:devctl NOT CONNECTED
However, the HBA ports are confirmed to be connected to switch ports:
ls -l /dev/cfg
c3 -> ../../devices/pci@8,600000/SUNW,emlxs@1/fp@0,0:fc
c4 -> ../../devices/pci@8,600000/SUNW,emlxs@2/fp@0,0:fc
fcinfo | egrep 'cfg|State'
OS Device Name: /dev/cfg/c3
State: online
OS Device Name: /dev/cfg/c4
State: online
Changes
Previously the HBA ports were connected to storage devices:
luxadm -e port
/devices/pci@8,600000/SUNW,emlxs@1/fp@0,0:devctl CONNECTED
/devices/pci@8,600000/SUNW,emlxs@2/fp@0,0:devctl CONNECTED
The connections were through two separate switches. Note the duplicate Port IDs connected to different storage array ports:
luxadm -e dump_map /dev/cfg/c3
