Symptoms
The output of "cfgadm -al -o show_FCP_dev" shows FC HBA port LUNs in failed condition.

Changes
LUN(s) mapped to FC HBA port no longer seen.

Originally,LUN 1 and 2 were seen:

But now luns are not seen:
