Symptoms
Running the format(1m) command causes the "Illegal Request" counter to increment in the output of iostat -En. No events of this condition are logged in the /var/adm/messages. Also, This condition shows up in kernel patch <Patch 150400-16> or <Patch 150401-16> or higher. Heres' an example:
Note the "Illegal Request" counter changed from 11 to 12.
Additionally, nothing shows up in the message logs... we only have these increasing illegal request counts seen with iostat
