Solaris Cannot Boot due to HW Issue With FC Tape Device

Symptoms
Customer is unable to boot Solaris 11.2 systems when tape library IBM SL3500 was configured on the SAN and seen by these hosts.
After an update to latest Solaris 11.2 SRU, this system would not boot and reported the following error messages, then transitions to maintenance mode.
Changes
On two different hosts, after updating Solaris, the customer was unable to boot on either the new or original Boot Environment (BE), so the Solaris update was not the problem.
