Symptoms
After upgrading to Solaris 10 U11 or Solaris 11.x, the server fails to boot, hanging on configuring the Emulex adapters installed on the server.
Case study:
Solaris 10 M4000 server with two LPe11002-S cards installed, running Solaris 10 U8.

Note that from the boot -v console above, the server hanged precisely when configuring
emlxs instance 3, the one showing the initialization errors.
