Zpool Suspended Due To Disk Errors After F/W upgrade (Doc ID 2018382.1)

Cu had ZFS pool FMA errors and needed to upgrade disk firmware.
After applying the disk F/W patch, the pool still showed "unavailable" even though the errors were cleared

Goal

Problem: after clearing fma errors and doing a disk firmware update CU still sees
