'Disconnected command timeout' messages observed on SAS targets 9 and 10

Symptoms
T5-2 server running Oracle Solaris 11.2.15.5.1
Server firmware version 9.5.1.b
Two internal SAS drives:
The following actions were done without success:
- The drives were upgraded to their latest firmware.
- The onboard mptsas chipsets were also upgraded to their latest revision.
- The system board was replaced as well as the SAS interconnect cables.
These troubleshooting steps didn't correct the issue. Also it was observed that little IO was being placed on the drives at the time of the failure, hence this couldn't be explained as a consequence of the workload on the server.
