Server Rebooted with panic:Evaluation:head write-back index larger than the last head index caused panic (Doc ID 2109567.1)

Goal

Server had a panic reboot.

Findings: noticed both interfaces (igb0/igb1) of one IPMP are failing over each other and when you hit the interfaces with medium-heavy traffic the server crashes and therefore reboots. See below for details:
