Symptoms
This issue occurs if an HBA is assigned to a guest LDOM using the SR-IOV feature.
Find below an example of this setup. It's worth mentioning that this doesn't occur for Emulex HBAs. Only Qlogic HBAs are affected.
On the control domain, two virtual functions are created from two different HBAs:
