Oracle HSM: QFS Shared Client With Distributed I/O Tape Status Issue

Customer reported that the samu 't' display does not correctly reflect tape status, showing tapes in a drive ready status that aren't even mounted anymore.
In addition they saw sam-scannerd reporting the following in sam-log:
