Solaris 10 Server Hangs During EMC XtremIO Disk Array Firmware Upgrade - ASC: 0x4 (<vendor unique code 0x4>), ASCQ: 0x1d (Doc ID 2168948.1)

Symptoms

During EMC XtremIO disk storage upgrade , Oracle CRS panics the Solaris 10 server01 as it cannot make IO to OCR Vote disks and crash dumps.
After restart, Customer was not able run their applications because of very poor performance.

These are two Veritas Cluster nodes (server01 and server02) with Oracle RAC and Solaris 10 (T5-8 servers)

From the SAN / FC connectivity point of view, the Solaris 10 T5-8 server has 8 16GB Qlogic FC HBAs Dual port,
a total of 12 ports connected to the SAN to access EMC arrays, XtremIO arrays and tape devices

More than 8000 LUNs mapped to this server, that correspond to about 1000 disks/LUNs from EMC VMAX-Symmetrix and XtremIO
each disk/LUN with 8 "different entries in format" or "paths" (as per vxdmp information)
