Exalytics - After Configuring Flash Using Configure_Flash.sh script and Rebooting Server "/u02" Mount Missing

Information in this document applies to any platform.
Rebooted server after configuring flash by running the Configure_Flash.sh script on an Exalytics, Release 18.104.22.168 server with Linux 6. After reboot the /u01 mount is missing.
df -h command below does not show "/u02" mount point:

On "/dev/nvme0n1” disk all partitions are lost as shown below:
