Symptoms

This is a Solaris 11.3 SRU 20.6.0 T5-2 server with two Oracle Qlogic FC HBA Dual port,
four ports connected to the SAN to access a SE6180 storage array

The problem is observed only with three new LUNs mapped from SE6180 with controller ports 20240080exxxxxxx and 20250080exxxxxxx
