Symptoms
A ZFS Storage Appliance running an OS release older than 8.6.12 reports DISK-8000-0X FMA 'predictive-failure' events in conjunction with a 'asc=0xb/ascq=0x97' code.
(note this also applies to other predicted fault ascq codes, i.e. 0x96; ANY predicted fault that isn't asc = 0x5d is considered 'false')
These asc/ascq codes can be identified in a Support Bundle (see below for alternative workflow method):

Newer disk firmware added the asc:0xb/ascq:0x97 code to indicate "warning, slow response".
