Oracle HSM: ACL Access Control Does Not Work Properly With a Group Count Greater Than 16

Symptoms
A customer running HSM 6.1.18 had a user who was accessing a SAM-QFS filesystem through SMB, shared out from a QFS client to a Windows client. That user belonging to 23 groups was randomly losing access to a directory (folder disappearing). After reducing the number of groups to 15, the user started to have access to the directory.
After changing ACL access control 'ngroups_max=32' but with a group count greater than 16, they reported still seeing file access problems, seeing 'Permission denied' errors.
