T4-1 - Unable To Get ILOM Prompt, SP is not responding or providing the sensor values

Symptoms
The customer reported:
- We are unable to get the ILOM prompt from the serial port in the T4-1 server.
- No matter we press #. any number of time, it is not going to -> prompt.
- Rear panel LEDs: Service Action Required LED is ON!
- Front Panel LEDs: Service Action Required LED is ON!
- After a power cycle, No SP boot log observed.
- It was bypassing the ILOM, but we could see the POST process and Solaris boot.
- No matter we press #. any number of time, it is not going to -> prompt.
- Rear panel LEDs: Service Action Required LED is ON!
- Front Panel LEDs: Service Action Required LED is ON!
- After a power cycle, No SP boot log observed.
- It was bypassing the ILOM, but we could see the POST process and Solaris boot.
From Explorer provided:
Even after power cycle, they could not upgrade the system firmware.
Changes
The server was a recycled one, that was earlier used in different data center.
