Install the latest driver and QConvergeConsole CLI.
QConvergeConsole CLI - Version 2.3.0 (Build 5) is the latest version
Updating to latest 11.4SRU10 (latest qlc driver) did not fix the issue.
Changes
Seems to affect only QLogic/Marvell 16Gb HBA (QLE8362 - P/N 7023303) and 32GB HBA (QLE2742 - P/N 7335902)
