HSM (SAM-QFS): T10000D Tapes Showing as Damaged Media Media and Media Blank Check With DIV On

Symptoms
Customer inserted new T10000D tapes. Most error out and now showing as 'Damaged Media'
Verified the tapes do not have duplicate barcode, not write protected, etc. Tested against all known good drives.
The 'new' tapes may have been previously used by other environments and have data written in.
Have carried out procedures such as tplabel -erase to try and deal with the issue.
Found the following tapealerts shown in the tape device logs (/var/opt/SUNWsamfs/devlog/[drive eq id])
