OCSM Install Failed

Goal
Upgraded the system to 4.2.0.0.0 and customer was not able to login to OCOM system.
The MySql log file shows the following error:
2019-11-06T17:50:18.687417Z 29 [Note] Access denied for user 'root'@'localhost' (using password: NO)
Reinstallation didn't help to clear the MySql error.
mysql.connector.errors.ProgrammingError: 1045 (28000): Access denied for user 'root'@'localhost' (using password: YES)
/opt/oracle/ocsm/usr/bin/plddb.py(22708) INFO: First attempt to access the database failed: 1045 (28000): Access denied for user 'root'@'localhost' (using password: YES)
/opt/oracle/ocsm/usr/bin/plddb.py(22708) INFO: Database does not exist, creating it...
/opt/oracle/ocsm/usr/bin/plddb.py(22708) INFO: Ignoring createdb.sql
Tried following the below document, but the issue remain same.
