VSM SL3000 drives do not come online. There is a suspicion of a problem with Brocade FC Switch.

The problem started after the switch firmware was upgraded to:

7.2.1b ▬► 7.3.1c ▬► 7.4.1e ▬► 8.0.1b ▬► 8.2.1
