Symptoms

The panic string will be: BAD TRAP

The panic stack will include either:

ssd_return_failed_command_no_restart
ssd_failfast_flushq
ssd_retry_command

or:

sd_return_failed_command_no_restart
sd_failfast_flushq
sd_retry_command
