Symptoms

After upgrade, Hitachi 2TB and 3TB drives repeatedly report the following in Fault Management Architecture logs (FMA) "ireport.io.scsi.cmd.disk.dev.rqs.derr". Further review of the FMA logs, resets with error codes "06/29/04 (Self Initiated Reset)" can be seen.

Changes

Hitachi models H7220AS60SUN2.0T and/or H7230AS60SUN3.0T with original code A142 were upgraded to A6C0.
