The panic string will be:
assertion failed: fm_flags_class == SCSI_FM_FLAGS_CLASS_FAILIO
The panic stack will include either:
scsi_buf_fm_report_post
ssd_fm_report_post_io
or:
scsi_buf_fm_report_post
sd_fm_report_post_io
